Sinosic Perosh Martial Arts complies with Privacy requirements as outlined in the Privacy Act 1988.
Sinosic Perosh Martial Arts collects personal information from Student Induction Forms, Membership Forms, and Direct Debit Forms. Sinosic Perosh Martial Arts holds personal information of its members such as personal information details and any medical conditions, which may prevent them from doing martial arts. Sinosic Perosh Martial Arts also holds bank account or credit card details of its members who pay their membership by direct debit.
The purpose of holding personal information of members is to:
- Keep them regularly informed of Sinosic Perosh Martial Arts’ operating hours, services and news
- Be able to contact the member
- Follow up with its members when the direct debit company rejects payments
- Use their name on Company Name’s website or newsletter
If members do not want to be sent any information they should contact Sinosic Perosh Martial Arts immediately to inform them by the contact details below.
Sinosic Perosh Martial Arts stores all the above information in a folder and is stored in a filing cabinet on premises. Sinosic Perosh Martial Arts also enters all of the information collected in to a database. Sinosic Perosh Martial Arts also sends the original direct debit forms filled in by the member to the direct debit company.
If a member has a complaint the member should hand in the complaint in writing directly to the chief instructor. Sinosic Perosh Martial Arts will then attempt to resolve the issue within 30 days. If the issue is not resolved in 30 days the member can make a complaint by sending their complaint in writing to the Privacy Commissioner at:
